Home Health QAPI: Maximizing Compliance & Quality Care

Regardless of how long you’ve been doing it, QAPI is not easy. Success takes time, an understanding of the concepts, and complete staff involvement. It’s worth every bit of the effort though; QAPI helps improve patient – and therefore agency – outcomes, which are so necessary in today’s pay-for-performance environment with five-star ratings and value-based purchasing. QAPI also will help your agency avoid the crisis du jour mindset – the daily ”putting out fires” grind - and instead fix the root of the problems! This will then free up the time needed to ensure your QAPI program is a good one. As you progress with quality indicators, your compliance with clinical documentation, processes, and home visits will also increase. This will lessen the risk of condition-level deficiencies, immediate jeopardy, and sanctions.

This webinar will address the importance of achieving a true interdisciplinary way of providing care, including a focus on team meetings. It will explore methods that utilize team strengths, leading to a cooperative approach to problem-solving. It will also cover ways to avoid the multiple-plans-of-care trap (“the nurse’s plan”, “the social worker’s plan”, “the chaplain’s plan”), and create plans that facilitate the understanding that all team members are responsible for all the goals within their scopes of practice. The bottom line and highest priority (supported by research) is that better care and higher levels of patient/family satisfaction can be achieved through effective interdisciplinary team functioning.

There is a growing segment of the population which has either never been a part of a faith community or has left their faith community due to spiritual crisis or changing belief systems. Chaplains and other team members need to be able to meet these patients and families where they are and find ways to address their existential concerns and their need to bring meaningful, peaceful closure to their lives. The problem is that so many of the “tools in our toolbox” come from supporting some kind of belief in a Divine Being. This webinar will explore the skills needed to provide spiritual care to those who experience and express their needs outside of the context of religious faith and belief.

This session will discuss practical, powerful ways to work through these challenges among our colleagues, clients, and industry partners to stop letting conflict destroy relationships and opportunities.

Agencies with strong administrative, clinical, and quality processes that directly support the hospice interdisciplinary group achieve outstanding survey outcomes, as well as ongoing compliance. While direct care clinicians are your hospice’s “frontline,” collaboration is essential to prevent deficiencies, and to develop, implement, and monitor corrective actions. This session will address the top 10 hospice survey deficiencies and the role of leaders and clinicians in minimizing deficiencies, evaluating current practices, and implementing tips and processes to ensure positive survey results.
