Under the mentoring and teaching of the Sr. Director of Clinical Operations, the RN Team Manager is responsible for upholding the role of learner, teacher, overseeing resource allocation, carrying out set standards, developing and coaching staff, and ultra-high accountability for creating a meaningful and powerful culture. Must have the ability to execute an effective, supportive, sensitive approach in response to the needs of patients, families and staff.  Must have the ability to flex schedule to oversee evenings, nights and weekend operations. In addition, this position acts as Administrator on-call on a rotating basis.

 

Qualifications

  • State of Florida RN License
  • Must have three (3) years of customer service and five (5) years of managerial experience, preferred.
  • MS Office Suite preferred. Netsmart EMR
  • Ability to make sound decisions and “see the whole picture”.
  • Ability to lead and teach an interdisciplinary
  • Must be able to implement and execute, coach and mentor the set Standards of the organization.
  • Strategic thinking, planning skills and the ability to interpret regulations and laws and disseminate information are

Provide program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments. Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development. Key Result Areas

  • Management of the Support Services Team: Crisis Care, On Call and 24-hour triage Call Center. Supervising and evaluation of staff.

 

  • Responsible for mastering, teaching, and adhering to and upholding agency-wide and clinical Standards and ensuring excellent customer
  • Perform ride-alongs 60% of the time with field
  • Educates, mentors and coaches’ staff to ensure excellent customer service, high level staff performance and clinical excellence as measured by positive feedback from Family Satisfaction Survey, QI team performance, integrity of staff
  • Responsible for the management and coordination of the day-to-day operations of patient care on the respective team. Outcomes measured by employee/client feedback, family outcomes/feedback, and employee performance levels. Works collaboratively with other leaders to accomplish this
  • Participate/provide leadership during the regular interdisciplinary team meetings, with oversight of the development, review and revision of the patient/family plan of care. Works with team to facilitate well organized productive and timely IDT
  • Coordinate the effective use of DME and
  • Effectively allocate resources by benchmarking and maintaining a budget based on Net Patient
  • Direct, monitor, and implement policies, procedures, and
  • Work collaboratively with peers, cross-functional team and internal
  • Responsible for the assignment of appropriate staff to provide care to patients and families, utilizing staffing standards to assure excellent end of life
  • Accountable for the oversight of
  • Must provide initially and update as appropriate all required information for credentialing at area healthcare facilities and hospitals and maintain good standing with
  • Detect deviations from set Standards and hold team members
  • Manage incidents and internal/external complaints in accordance with Treasure Coast Hospice
  • Teach and uphold Treasure Coast Hospice’s standards with a focus on perfect documentation, perfect visits, perfect communication, and professional dress &
  • Lead, develop and motivate employees with confidence, energy, integrity and
  • Participate as an active supporter of the Treasure Coast Hospice compliance program including the emphasis to report, as well as complying with all Federal, State, and Local laws, as well as the organization’s compliance
  • Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and
  • Administrator on-call on a rotating basis

Position will be categorized as category III (involves no routine or potential exposure to blood, body fluids/tissues).

 

Working Conditions
