Although you may be cognizant of the importance of documentation in supporting payment, don’t allow that to overshadow its two other important functions: creating a real-time record accessible by all IDG members and, although rarely considered, building confidence in care. When is documentation completed in your hospice? As part of the visit? If not, why not?
