SUMMARY
On Call Nurse Coordinator is responsible to triage calls and direct the client or caregiver as to what is the next appropriate action steps per protocol during a medical emergency.

PRIMARY FUNCTIONS
• Responds to after-hour inquiries from alternate caregivers or staff members.
• Provides advice and emotional support to patients, family members, or appropriate parties seeking medical guidance.
• Ensures appropriate information is given based upon level of emergency.
• Maintain accurate patient medical records
o Upload On call information in NetSmart or other information sources.
o Create notes in NetSmart for IDG staff.
• Administer nursing care to terminally ill patients in a home care setting
• Diagnose and establish patient treatment plans
• Communicate with collaborating physicians or specialist regarding patient care
• All other duties as assigned.

SUPERVISORY RESPONSIBLITIES
This job has no supervisory responsibilities.

QUALIFICATIONS/ COMPETENCIES
(EDUCATION and/or EXPERIENCE)
• Hospice/Palliative Care experience preferred.
• Clinical background with Bachelor Degree preferred.
• At least 2 years of RN exp.

Ability to read and comprehend instructions, correspondence and memorandums; ability to write correspondence. Ability to effectively present information to vendors, clients, and other employees of the organization. Ability to listen and empathize with others and to refer appropriate sources for intervention.

MATHEMATICAL SKILLS:
Ability to accurately add, subtract, multiply and divide in all units of measure using whole numbers, common fractions and decimals. Ability to compute rate, ratio, percent, and to draw and interpret bar graphs.

REASONING ABILITY:
Ability to apply common sense understanding to carry out instructions furnished in written, oral or diagram form. Ability to deal with problems involving several concrete variables in standardized situations.

CERTIFICATES, LICENSES, REGISTRATIONS:
Valid State of Florida RN License and CPR Certification.
