Description

Sat & Sun 10am - 10:30pm and Friday 5pm-1:30am. - Primarily dedicated to north, however can be asked to visit Desoto and Charlotte counties in addition to Sarasota and Manatee service areas.

Tidewell Hospice, the sole provider of hospice services in four counties of sunny Southwest Florida, is seeking an experienced and team-oriented RN Triage Case Manager.

The RN Case Manager creates and maintains a plan of care, serving as the coordinator for patient and family interaction with Tidewell for the duration of the patient’s hospice experience. When a patient and loved ones think of Tidewell Hospice, they will think first of the RN Case Manager. The case manager must be able to listen, observe, communicate and sensitively provide nursing services in emotionally intense situations.

The RN Case Manager:

  • Reviews and updates the care plan, physician orders, pharmacy profile and other records.
  • Educates patients, family members and other caregivers about the disease process and decline, available palliative interventions, the dying process and safety practices.

Requirements:

  • Applicants must be a graduate of an approved nursing program and own a current Florida RN license.
  • BSN or MSN preferred.
  • Minimum one year experience in an acute-care hospital environment providing direct patient care; previous experience as a hospice RN preferred.
