The Hospital Admissions Registered Nurse (RN):

• first responder who assesses eligibility for the Continuum of Care Programs within Suncoast Hospice and Empath Health while providing optimal access in a hospital setting

•will also act, in concert with the Professional Liaison, as a consultant to the hospital to educate them about Empath Health services, access to hospice care and delivery of end of life care

•expert in end-of-life services, as well as adept at marketing and educating the professionals at the hospital

•expected to identify and facilitate opportunities for an increased Empath Health presence and collaborations which will improve access to hospice care and related programs by providing the right care at the right time depending on specific patient needs, program specifics and program guidelines

•leads the Admissions clinical presence in the hospital, coordinating with other Admissions RNs, the Professional Liaison, the Patient Concierge and any other Empath Health employees working in the hospital to ensure timely response and access to hospice services

Position Requirements

Education and/or Experience: Associate’s degree (A.A.) or equivalent from two-year college or technical school; Minimum of or six months to one year related experience and/or training; or equivalent combination of education and experience.
• Licensure:
• Valid Florida license as a Registered Nurse
• Valid Florida State Driver’s License
