Stratum Life PACE, a member of Empath Health is currently seeking a Physical Therapist to join our team!
The Physical Therapist:
- Under the supervision of the PACE Rehab Manager, provides physical therapy services to participants of Suncoast PACE.
- Provides clinical physical therapy evaluation and treatment for program participants, on acute, restorative or maintenance levels as needed at Day Center, ALF, Nursing Home or in-home setting.
- Integrates the physical therapy treatment plan into the Interdisciplinary Team comprehensive plan of care.
- Assists in the ordering, inventory, distribution and maintenance of durable medical equipment for participants.
Requirements:
- Education: Be a graduate of a physical therapy curriculum. Current State of Florida Physical Therapy license required.
- Minimum: Bachelor’s, Preferred: Master’s
- Minimum one year of experience working with the frail or elderly population
- Intermediate proficiency in MS Word, Excel and PowerPoint, MS outlook.
- Be a graduate of a physical therapy curriculum approved by one of the following: The American Physical Therapy Association; The committee on Allied Health Education and Accreditation of the American Medical Association; The council on Medical Education of the American Medical Association and the American Physical therapy Association required. Other organizations approved by the Secretary.
- Valid CPR certification from the American Red Cross or American Heart Association.
