Palliative care program:

Palliative care is provided in the form of consultations as an additional layer of support working with the patient’s doctors.  Palliative consults are available at hospitals, nursing homes, assisted living facilities and private residences. The Haven Care Team provides expertise in managing complex pain and symptom issues as well as providing spiritual and emotional support during a difficult time.

Veteran Program: We Honor Veterans (Level III)

Haven celebrates and recognizes the contributions of veterans in the communities we serve.  We strive to partner with community organizations to better recognize and serve our veterans.  At Haven, our clinical staff participates in continuing education focused on veteran’s needs.   In addition to education, we utilize a military history checklist with our veterans upon admission to ensure we know and understand their service history.  We use this information to design a unique plan of care to address any concerns that may arise from our patient’s military service. Perhaps the most emotional part of our We Honor Veterans Program is the vet-to-vet pinning ceremony for those veteran patients who wish to be recognized.

Pediatric program:  Pegasus – Haven offers pediatric support and provides medical, emotional and spiritual care for infants, children, young adults (to age 21) and their families who are faced with serious illness through the Pegasus Program. Haven’s family-centered care is based on a comprehensive plan that respects the child’s and family’s needs, beliefs and wishes. The goal is to help infants, children, young adults, their families and others who are meaningful in that young person’s life to live every day as fully as possible. Children enrolled in the Pegasus Program can continue to seek aggressive and curative treatments for their serious illness and also receive hospice services. The young person and their family do not have to give up hope for a cure, and at the same time, they receive all of the benefits of a hospice program.

Hospice goes where the patient is. Whether or not there is an administrative office in your city does not determine the hospice providers area of service.
