What are Hospice-Veteran Partnerships (HVPs)?  HVPs are coalitions of VA facilities, community hospices, state hospice organizations, and others working to ensure that excellent end-of-life care is available for our nation’s veterans and their families.  These community-based or statewide coalitions function independently or within an existing structure, and establish an enduring network of hospice and VA professionals, veterans, volunteers, and other interested organizations.

HVPs strengthen relationships between community hospices and VA facilities.  When a hospice gets involved in the We Honor Veterans partner program, they learn to recognize the unique needs of veterans and their families and how to accompany and guide veterans through their life stories toward a more peaceful ending.  This webinar will provide an overview and best practices for a successful We Honor Veterans partnership and other VA partnerships that will increase access to hospice and palliative care for veterans and honor the commitment of serving those who have served us.
