Role: Day to Day responsibility for ensuring the house is clean and sanitary, properly clean and sanitizes patient rooms and common areas, bathrooms, offices, all other spaces as assigned.

 

Qualifications:

  • Prior related experience in hospital, hospice house, nursing home, and/or general cleaning experience as deemed necessary by the EVS Manager for the position.

Daily Responsibilities:

General:

  • Follow written job flow on daily basis to ensure cleanliness and sanitation of all parts of building
  • Remove Trash and/or cardboard to exterior of building
  • Share expertise with co-workers both formally and informally
  • Ability to constantly lift, pull, carry, and/or push objects weighing up to 55 pounds
  • Ability to stand, kneel, walk, climb, crouch or bend for long periods of time
  • Employee carries out detailed instructions for several tasks.  Guidance and instruction is always available to the incumbent and prior permission is required before changing the methods or steps used in the job’s work.
  • Perform assigned housekeeping care to various types of surfaces, floors, equipment to include, but not limited to, vinyl composition tile, ceramic tile, quarry tile, concrete, terrazzo, hardwood flooring, rubber, carpet, painted wall surfaces, wall hangings, linens and towels, beds and all components thereof, laminate wood, hardwood TV stands and tables, radios, plastic and metal carts.
  • Determine needed supplies and equipment, and relate that need to immediate Supervisor
  • Sweep, mop, strip, dust, and refinish floors
  • Carpet cleaning to include vacuuming, shampooing, and extraction
  • Wood floor cleaning to include spray buffing and oiling as directed
  • Move furniture and small equipment to clear work area
  • Account for equipment and materials assigned
  • Assist in training new employees for housekeeping dept.
  • Responsible for cleaning all equipment used thoroughly after each use
  • Skill in the use of and care for housekeeping equipment including but not limited to vacuum, dusters, chemical pumps, etc.
  • Report any injuries, both personal and witnessed, immediately to the Supervisor
  • All other duties as assigned, by management for Chapters Health System.

 

Customer Service and Marketing:

  • Assure timely attention to effective solutions for customer concerns and complaints.
  • Immediately report any customer service complaints or concerns to Dietary Coordinator and/or EVS Manager.

Role: The Palliative Care Assistant has responsibility for administrative and general supportive clinical duties, under the direct supervision of the Palliative Care Director.  S/he will provide support to medical and other clinical staff within Chapters Palliative Care and will facilitate completion of provider credentialing, payor contracting, and contracting with partnered facilities.  S/he must be able to demonstrate effective interpersonal skills and an understanding of the interdependent roles of staff and various allied health professions.

 

Qualifications:

  • A  minimum of a high school degree, college degree from an accredited university is preferred
  • A minimum of two years of administrative experience in a clinical setting  
  • Valid Florida driver’s license and automobile insurance as per Company policy

Job Responsibilities:

  • Responsible for scheduling Chapters Health Palliative Care medical and other clinical staff for patient/clinic visits in all settings.
  • Schedules and verifies patient appointments for outpatient clinic.
  • Facilitates all Chapters Health Staffing requests and schedules providers as needed.
  • Enters palliative care referrals and admissions into appropriate electronic medical record.
  • Assists with insurance verification as needed and as requested by the Medical Services Practice Manager.
  • Completes and submits monthly administrative timesheets by Chapters Health Palliative Care clinician.
  • Communicates with providers regarding billing errors (missing charges, wrong POS, patient discharges, Event Audit Report to identify if patient enrolled in Hospice Program) and provides support to providers in rectifying errors.
  • General administrative duties, including coordination of meetings, communicating schedules and information from palliative care program.
  • Enter and distribute information as needed for executive or departmental dashboards and QAPI reports.
