Role:

The role of the Director of Patient Access is to provide management direction and leadership for a dynamic department whose primary responsibility is to ensure the timely access to hospice care for patients and families. This responsibility includes the efficient intake and scheduling of referrals, as well as the evaluation and assessment of patients to determine eligibility for hospice admission. The Director is also responsible for establishing and maintaining strong relationships with care partners, ensuring that their needs and concerns are met and resolved. The Director oversees the performance of the department, as measured by established metrics and benchmarks. The role is a member of the leadership team, and as such, works collaboratively with the Regional Directors to ensure that strategic initiatives and business opportunities are met.

 

Qualifications:

  • Minimum of five to seven years relevant experience in admissions or other health care administration function with three to five years hospice, home health or other health care experience preferred.
  • Master’s degree business, marketing or in a health related field required.
  • BSN/MSN preferred
  • Five to seven years of management/supervisory experience managing a team of more than fifty in a fast paced environment.
  • Must have valid driver’s license and automobile insurance per Company Policy and Procedure.
  • Ability to travel to off-site locations*
  • Ability to manage physical demands and emotional stress of workload.

Admissions Clinical Coordinator

Position Requirements

  1. Minimum of two years Hospice experience. Hospice Admission/Management experience a plus.
  2. Seven to ten years total experience in related discipline. Current licensure if applicable.
  3. Masters degree in one of the core disciplines of Hospice preferred.
  4. Excellent problem solving, organizational, written and verbal communication skills a must.
  5. Extensive knowledge of Hospice services and programs.
  6. Extensive knowledge of reimbursement.
  7. Extensive knowledge of community resources.
  8. Ability to direct individual staff and team toward the provision of the highest quality patient care services.
  9. Must have reliable transportation and must be able to provide proof of automobile insurance.
  10. The successful candidate must be willing to submit to pre-employment assessment of keyboarding skills.
