American Academy of Hospice & Palliative Medicine:
The American Academy of Hospice and Palliative Medicine (AAHPM) is an organization of physicians and other medical professionals dedicated to excellence in and advancement of palliative medicine through prevention and relief of patient and family suffering by providing education and clinical practice standards, fostering research, facilitating personal and professional development, and by public policy advocacy.

The Center for Hospice, Palliative Care & End-of-Life Studies: at USF unites a major research university with community providers of hospice and palliative care in an equal partnership aimed at influencing policy and practice through research and education. The Center began informally in 1996, and was formally recognized by USF in 2000. The Center brings together members from hospices across West Central Florida, H. Lee Moffitt Cancer Center and Research Institute, James A. Haley Veterans Medical Center , and the University of South Florida . By facilitating collaboration among experts in end-of-life care who represent multiple disciplines, The Center is able to generate cutting edge research that addresses issues in hospice, palliative care, and end-of-life studies from a comprehensive perspective.

Florida Department of Elder Affairs: The Florida Department of Elder Affairs is the agency constitutionally designated by Florida voters to "serve as the primary state agency" responsible for administering human services programs for the elderly. (Section 430.03 Florida Statutes). Its purpose is to serve elders in all aspect possible ways to help them keep their self-sufficiency and self-determination.

Florida Department of Health: To promote and protect the health and safety of all people in Florida through the delivery of quality public health services and the promotion of health care standards.

Florida Prescription Drug Website: This website was developed by the Florida Attorney General and the Agency for Health Care Administration (AHCA) to help consumers shop for the lowest price in their area for their prescription drugs. The Florida Prescription Drug Price website provides pricing information for the 50 most commonly used prescription drugs in Florida. The prices are the “usual and customary prices,” also known as retail prices, reported monthly by pharmacies. This is the price that an uninsured consumer, with no discount or supplemental plan, would normally pay. Prices at your local pharmacy may change daily, so this website is only meant to help you compare prices at different pharmacies and are not a guaranteed price. GetPalliativeCare.org: The Center to Advance Palliative Care introduces getpalliativecare.org, a palliative care resource created for consumers that also includes sections for medical practitioners and policymakers. This new website is easy to navigate and provides clear, comprehensive palliative care information to patients and families coping with serious, complex illness.

Growth House: A non-profit organization that hosts support groups dealing with death and dying and provides links to a wealth of end-of-life information.

Health Proxy Form:
This is Florida’s recommended health proxy form. 

Hospice and Palliative Nurses Association: Established in 1986, the Hospice and Palliative Nurses Association is the nation's largest and oldest professional nursing organization dedicated to promoting excellence in pain management and end-of-life care.

Last Acts: Dedicated to improving end-of-life care through sharing of ideas and solutions by professional care givers, institutions and individuals.

Living Will Form: This is Florida’s statutorily recommended Living Will form.

National Hospice and Palliative Care Organization: The only non-profit public benefit organization devoted exclusively to hospice care. NHPCO is dedicated to promoting and maintaining quality care for terminally ill persons and their families and to making hospice an integral part of the U.S. health care system.

Project GRACE: A non-profit education organization that provides education programs and resources in end-of-life medical care and advance care planning. Project GRACE offers a medical-scenario specific Advance Care Plan document.

Project on Death in America:
The Project on Death in America (PDIA), an Open Society Institute initiative, has closed. From 1994 to 2003, PDIA worked to understand and transform the culture and experience of dying and bereavement. Over the course of nine years, PDIA created funding initiatives in professional and public education, the arts, research, clinical care, and public policy. PDIA and its grantees have helped build and shape this important and growing field, and have helped place improved care for the dying on the public agenda.
