Clerical Help
Part time Clinical Support Specialist, knowledge of a variety of software systems and experience with medical supplies, records and terminology is necessary. Quality Assessment/Infection Control Nurse
Part time 20 hours/week. Responsible for compliance and quality assessment audits; on-going surveillance of infection control indicators; annual review and revision of infection control manual including exposure control plans; monitoring biomedical waste management processes. Works closely with Education dept to identify training needs. The successful candidate will possess RN licensure, experience with infection control processes preferred.

Community Relations Assistant
FT with some evenings required. Responsible for providing support to the CR Dept and assist with special events, tracks media coverage and handles correspondence. Must be detail oriented with the ability to prioritize and multi task with minimal direction. AA degree or equivalent work experience required.

Caring Tree Counselor
Full-time Bereavement Counselor for Caring Tree Program. Duties include providing psycho-educational grief support to youth and caregivers in schools, at Big Bend Hospice, and in the community. Master's Degree in Social Work, Counseling or other related field. Excellent communication, public speaking, group work, and organizational skills required. Ability to work in a close-knit team. Occasional night and weekend work required.

Triage Nurse (RN) – Full Time with benefits, 1:00am-9:00am, weekend availability and flexible schedule required.  This position is responsible for providing after hours service to ensure that patient calls are received and routed to the correct person and that the appropriate care is given in a timely manner as necessary.  Minimum of 1-2 years experience in triage or other nursing competency, or equivalent combination of education and experience required.  Current Florida RN license required, CHPN preferred.
               

Inpatient ARNP – Full Time with benefits, shifts vary, some on-call required.  This position is responsible for evaluating and coordinating patient/family needs to enable preventative and supportive care under supervision of the Chief Medical Officer.
Minimum of 2 years experience as an ARNP, graduation from an accredited school of nursing, current Florida Registered Nurse license and current Florida license as an ARNP required.  BSN or Masters Degree in Nursing or a related field preferred.  BLS required.  Minimum of 2 years of medical nursing experience, hospice and/or critical care experience preferred.  Experience with symptom and pain management and CHPN certification preferred.

Inpatient Registered Nurse – Full Time (3 nights per week, 7:00pm-7:00am), Part Time, (2 nights per week, 7:00pm-7:00am).  This position is responsible for identifying patient/family/caregiver needs and providing nursing and supportive care in accordance with the plan of care, and policies and procedures.  Current Florida RN license required, minimum of three years of nursing experience, and BLS required.  BSN, CHPN, Hospice and/or critical care experience, and experience with Health Information Management software preferred.                             

Inpatient Certified Nursing Assistant – Full Time (3 days per week, 7:00am-7:00pm), Part Time (2 days per week, 7:00am-7:00pm).  Weekend availability required.  Under the supervision of RN, this position is responsible for providing personal care to assigned patients in the Inpatient Unit, based on the home Health Aide Plan of Care as developed and updated by RN.  This includes bathing and assisting with ADL’s as appropriate.  Current Florida CNA certificate and three years of patient care experience required.  CHPNA, BLS, hospice and/or critical care experience preferred.  Experience with Health Information Management software is also preferred.                       

Driver/Technician - Full Time with benefits, normal office hours (8:00-5:00, M-F), some after hours required.  This position is responsible for the general maintenance, cleaning and repair of all home medical equipment that is utilized by patients of Hospice of the Comforter.  High school diploma or General Education Degree (GED), a current valid Florida Driver’s license and the ability to lift a minimum of 90 pounds is required.  A minimum of one year experience in warehouse operations or home medical equipment is preferred.

Administrative Assistant (CNAs) – Full Time with benefits, 32 hours per week,

Chaplin
PRN availability. Coordinate with the Triage team, provides spiritual care services for patients/families during the non-business hours on weeknights and weekends. Provides spiritual counsel for patients and/or families in spiritual distress and support for families at the time of a patient's death. Contributes to the smooth function of services for a 24 hour/7day a week hospice care program.
Master's Degree from an accredited seminary or school of theology.
Minimum five (5) years experience in pastoral care, at lease one (1) year of which was in health care setting. Minimum of one (1) Unit of Clinical Pastoral Education required, with advance certification preferred. FL Drivers license with driving record acceptable to Community Hospice insurance carrier, and reliable transportation

Psychosocial Specialist
PRN availability. Acts as part of the interdisciplinary care team and manages social service needs of a caseload of patients and families.
Coordinates all social service interventions and referrals, including needs assessment, supportive counseling, networking, resource procurement. Contributes to the smooth function. MSW from an accredited school of Social Work and three (3) years direct service experience, attends the Interdisciplinary Care Team (ICT) and advocates for patient/family needs with the team and community at large.

Associate Medical Director
Must be a Florida Licensed Physician who has experience in the practice of inpatient hospital medicine.  S/he will provide physician services to patients of Community Hospice, shall be a member of the Interdisciplinary Team (IDT) of a Hospice In-Patient facility and work closely with the IDT members, patient/family and the primary or referring physician to ensure quality care for the patient and family.  Typical qualifications would be equivalent to Doctor of Medicine or Osteopathy degree from an accredited medical school with a current Florida license, must be eligible for, or have membership in good standing on the medical staff of at least one of the accredited hospitals in the Community Hospice service area., Hospice and Palliative Medicine Certification by the American Board of Hospice and Palliative Medicine is desirable within 24 months of hire (if not already certified), experience with oncology, home health or hospice care delivery is desirable.

Advanced Registered Nurse Practitioner
This position will provide independent primary care to hospice patients in the inpatient setting for their general medical illnesses.  Though these are patients with terminal illnesses, the focus of this provider will be to render for unassociated medical illnesses separate from their terminal diagnosis.  Masters Degree with PNP certification, current state of Florida ARNP license, strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community, ability to maintain quality, safety and/or infection control standards, ability to work flexible hours when needed.

Director of Information Technology
This position will plan, direct and manage the activities of the information technology department to ensure alignment with overall business needs, strategic plan, budget and policies of the organization.  BS degree in Computer Science and/or a minimum of 7 years work experience in managing one or more major information processing functions, Masters degree preferred, 3-4 years of management and staff supervisory experience, comprehensive knowledge of hardware, software, networking, project and Process Management skills and techniques.

Call Center Manager
This position will organize and supervise the Referral Department and Follow-up Program staff to improve the processing of referrals to admissions in a patient-driven, cost effective, efficient and effective manner. Responsibilities include hiring, supervision, evaluation of staff, coordination of team activities and identification and support of call center technologies.  College degree or applicable work experience, minimum of 3 years supervisory or management experience, 3-5 years experience in a medical or hospice environment preferred.

Major Gifts Officer
This position will develop relationships with donors and volunteers that result in visibility and financial resources to support our mission and services. Plan and conduct, with the support of others, specific campaigns for major gifts.  Directly cultivate and solicit major gifts from individuals and organizations.  Identify, cultivate and secure grant opportunities.  Bachelor's Degree required plus 7 years of related fundraising experience including proven track record of direct cultivation and solicitation of major gifts from individuals and organizations. Planned giving experience and CFRE preferred, self starter with demonstrated ability to work independently, strong verbal and written communications skills, excellent organization, interpersonal skills, and presentation skills.

Intake Facilitators
Call center representatives will perform a wide range of activities pertaining to the accepting and processing referrals and dispatching Admission RNs. Coordinates assessment appointments and personnel; uses independent judgment within the framework of established policies and services.  Minimum requirement of a high school diploma; AA or Bachelor's degree preferred, 6 months experience in medical call center environment, or in a health care setting, good written and verbal communication skills with strong customer service orientation, computer knowledge (experience with patient/client data/software, computerized voicemail and paging experience preferred).

HomeCare Manager
The Homecare Manager utilizes management skills to facilitate provision of care in HomeCare.  Under physician's orders, per the Physician's Plan of Treatment, the manager is responsible for identifying and coordinating patient/family needs and for providing palliative, preventative, and rehabilitative care to the patient/family unit. The manager utilizes assessment, intervention, and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit. The manager is a mentor, teacher,  and preceptor to new staff and team members while providing home health care. The manager participates in quality management initiatives and financial management for the certified and non-certified patients.

Requirements:

1. Current Florida Registered Nurse license, BSN preferred.

2. Minimum of three (3) years nursing experience; with at least one (1) year acute care setting.

3. Minimum of two (2) years home health experience with thorough understanding of State and Federal Home Health Agency Regulatory Standards.

4. Minimum of two (2) years home health management experience with supervisory responsibilities

5. Interest, experience and expertise in the care of patients with chronic and/or life limiting illnesses.

6. Intravenous therapy training and experience.

7. Must have reliable transportation which enables them to perform tasks and responsibilities in a timely and appropriate fashion. Must be able to provide proof of automobile insurance, a copy of which will be placed in the employee file.

Medical Records Clerk/Information Services Clerk
The Medical Records Clerk is responsible for assembling, analyzing, and maintaining medical records according to established procedure.

Responsibilities:
• Process admission paperwork including chart assembly
• Request prior medical records from hospitals and physicians as indicated on the Nursing History and Physical and Team Daily Report
• Enters required information in the computer for the purpose of tracking medical record documents
• Files medical record documentation in a timely manner making certain that the patient name and number correlate to the chart
• Pulls and prepares charts for team meetings. Ensures that filing is current
• Pulls charts for audits, research and other legitimate requests
• Audits charts following the death/discharge of the patient. Identify delinquent documentation that is needed for chart completion
• Prepares and updates an audit list weekly, distributing the list to the team and department head
• Monitors the return of all documentation until the chart is complete
• Daily opening, logging and sorting of mail and team paperwork
• Maintains patient confidentiality at all times

Other Functions:
• Performs other job-related duties as requested.*

Manager Of Volunteer Services
The Manager of Volunteer Services provides leadership and direction for the company’s volunteer program, ensuring effective integration of the volunteer component throughout the organization and community. Evaluates Volunteer Services outcomes, identifies and promotes solutions and evaluates practice standards. Conducts formal and informal educational programs/experiences of Company members to support the Company Volunteer Services program. Maintains own personal knowledge and skills as an expert and role model in hospice volunteer practices. Supervises the Volunteer Coordinators and Volunteer Department Secretary.

Responsibilities include the development of strategic direction of the Volunteer Department and communicate it to all staff and volunteers; supervising Volunteer Services Coordinators, providing direction for recruitment, training, management and retention of volunteers for various Company programs; overseeing volunteer recruitment and screening and development and maintenance of volunteer training manual in collaboration with Volunteer Program Director. Will also perform additional duties as requested by the Chief Clinical Officer.

Qualifications:
• Degree in a human services related field required.
• 2 years experience in a health care setting.
• 2 years supervisory experience required.
• Excellent oral and written communication skills, including public speaking.
• Well developed interpersonal skills: ability to be tactful and understanding.
• Must be able to travel to resource centers and attend functions on evenings and weekends.

Health Information Supervisor (HIS)
The Health Information Services (HIS) Supervisor will be responsible for the management of day to day departmental operations including coordination and performance of all health information functions, document imaging, and direct supervision of department personnel.

Responsibilities include the day-to-day supervision and support for HIS Clerks, the implementation and maintenance of medical record systems that meet or exceed compliance with Federal/ State regulations, Joint Commission and health information standards; determining agenda and conducting staff meetings for all HIS staff including weekend or evening on-call, with minutes available for review; attending hospice in-services and outside programs/seminars and participating in professional or specialty organizations through membership and attendance at local meetings; planning, organizing and coordinating regular monthly department meetings with Director. Will participate in budgetary planning, preparation and control for the department with the Director of HIS and will be accountable for overall financial management of HIS personnel.

 Qualifications

Psychosocial/Bereavement Program Director
Candidate will be the subject matter expert, responsible for providing corporate level oversight of Psychosocial and Bereavement services for LifePath Hospice (LH) and Good Shepherd Hospice (GSH). Candidate will promote psychosocial and bereavement practices and excellence in hospice care by assuring that standards are met and affirm and assist in ensuring that quality psychosocial and bereavement services are provided to LH/GSH patients and families. Must possess Master's degree in Social Work (MSW) and current Florida Clinical Social Work License.

Internal Audit Director
The selected candidate will be responsible for developing and directing a comprehensive internal audit plan to ensure the Company’s polices and procedures are in compliance with applicable laws and regulations; conducting special investigations as requested; making recommendations for improved controls, operating procedures, accounting records and systems designs; and, providing reports and updates to Senior Staff and the Audit Committee on matters of policies and procedures. This position reports to the President and Chief Executive Officer.

The ideal candidate will be a CPA and/or CIA; have a minimum of 5 years relevant auditing experience (preferably in a healthcare organization subject to AICPA’s Health Care Audit Guide); and a Bachelor's degree in Accounting. Working knowledge of computer technology to maintain records, access/analyze data and generate reports is imperative.

ARNP
LifePath Hospice is seeking an ARNP for home visits in Hillsborough County. As an ARNP, you will be based out of our Tampa or Sun City office and provide primary and consultative care to our patients under protocol of the medical services team and our Medical Director, and participate in diagnostic and management decision making while promoting quality-of-life for our patients and caregivers. Requires a Master’s degree in Nursing, ARNP certification and licensure in the state of Florida as an ARNP.

Will be a self-directed Master's prepared RN to be responsible for supporting nursing practice with a specific focus in palliative care and pain/symptom management and promoting excellence in hospice care. The successful candidate will have experience in teaching and program planning. Pediatric experience is a plus. This autonomous advanced practice role provides educational allowance and requires rare weekends.

Regional Director
The successful candidate will provide oversight for delivery of patient care and community programs within a specific regional area.  Responsibilities include: leading interdisciplinary teams to deliver the highest quality of care to our patients and families; ensuring staff meet education, competencies, performance and compliance standards; provide leadership in the development, implementation and attainment of the Company’s strategic goals; and overseeing utilization of resources and budget accountability. Position reports directly to the Chief Clinical Officer for the program.
 
Requires a Florida RN license with BS degree; minimum 5 years progressive management experience in a healthcare setting with at least 2 years at the director level; budget and fiscal responsibility; prior hospice or home healthcare experience; and experience in care delivery systems redesign and integration. Master's degree in nursing, business or health care administration preferred.
