AVOW HOSPICE
Serving Collier County since 1983. We are Collier County’s original – and only – nonprofit hospice. We care for those who are terminally ill and support their friends and families through bereavement. Our Frances Georgeson Hospice House, built with community donations, is the only freestanding hospice inpatient facility in our county. Assistant Vice President of Giving
The Assistant Vice President of Giving is responsible for providing the overall vision and leadership in designing a major gift/planned giving fund-raising plan, laying the foundation for the next capital campaign by building the base for future growth, orchestrating the work of the professional staff and Board leadership to this end.  The Assistant Vice President is an active fund raiser with personal responsibility for managing a portfolio of donor relationships aimed at securing six and seven-figure gifts.  This position bears the ultimate responsibility of achieving annual fund-raising goals and will serve as an active member of senior management team to ensure integration of priorities as it relates to the philanthropy program. 

The AVP of Giving will:

develop annual and long-range strategic fundraising plans

Under physician's orders, the Hospice Nurse is responsible for identifying the physical and emotional needs of the patient/family and for coordinating the care necessary to meet these needs in order to maximize patient comfort and to enhance the quality of life for the patient/family.  Contribute to the development of an Interdisciplinary Treatment Plan in collaboration with the multi-disciplinary team providing a comprehensive plan of care.  Valid FL RN license required. BIG BEND HOSPICE, INC
Big Bend Hospice is a private non-profit organization which has served patients with a life limiting illness and their families in the eight counties of the Big Bend since 1983.    Big Bend Hospice provides physical, emotional, spiritual and practical care specifically tailored for each family.   Together with the patients own physician, the Hospice team of nurses, family support counselors, home health aides, music therapists, chaplains, and volunteers provide expert pain and symptom control.  Big Bend Hospice remains with each family for up to a year following the death of their loved one to provide support through the grieving process.  There are grief support programs for adults, children and teens.  All of our grief support programs are available to anyone in the community regardless of whether they have used Hospice services. Big Bend Hospice House provides care for patients with acute medical needs which require 24 hour medical supervision.

Clinical Social Services Specialist - Pool

The Clinical Social Services Specialist (CSSS) provides and coordinates a wide range of social services to patients and their families for HPC Healthcare, Inc. (Company). In concert with the interdisciplinary care team, the CSSS provides support to patients and their families to help them cope with illness, death, and/or loss. CSSSs work with patients/families to help them manage the physical, social and emotional aspects of illness, death and dying. The CSSS coordinates community resources to assist patients/families with their biopsychosocial concerns and provides services in a variety of settings, to include: home care, acute care, inpatient and residential hospice care, long term care, after-hours and respite care.

The Chaplain provides a wide range of spiritual services to patients and families enrolled in any of the Company programs. The Chaplain, in conjunction with other interdisciplinary team members, evaluates spiritual needs on a case-by-case basis so that appropriate spiritual care can be provided. At all times the Chaplain works closely with the interdisciplinary group to identify and problem solve spiritual care issues. Spiritual care and services may include Chaplain visits, contacting community clergy, enlisting a volunteer to meet spiritual needs, and/or conducting memorial and funeral services. The Chaplain also provides spiritual support to the staff and volunteers of the Company. This support may include activities and events such as holding memorial/funeral services, participating in staff support and individual ministry related to patient/family care. Staff Chaplain –
Typical qualifications would be equivalent to:  Ordination as a clergy person from a recognized religious community and demonstration that the minimum educational requirements for such ordination have been achieved.  Master’s Degree from an accredited seminary or school of theology.  Minimum five (5) years experience in pastoral care, at lease one (1) year of which was in health care setting.  Completion of a supervised chaplain education/training program, such as Clinical Pastoral Education (CPE), with at least four units (one year) of CPE.  Current State of Florida drivers license with driving record acceptable to Community Hospice insurance carrier, and reliable transportation.

Manager of Spiritual Care –
This certification requires a college degree, a master’s level theological degree or equivalent, clinical training in chaplaincy, ordination or commissioning for ministry by a recognized religious group, a current endorsement for Chaplaincy by a recognized religious group, and appearance before a national certifying commission for assessment of competency, Current State of Florida drivers license with driving record acceptable to Community Hospice insurance carrier, and reliable transportation. 

Director of Human Resources –
Masters degree from an accredited college or university and five to six years of related exPrevious hiring/firing and interview experience, benefit administration experience and Human Resource administration experience.  Five years previous supervisory experience in Human Resources Management. Knowledge of Principles, practices and procedures of Human Resource administration.  General overall working knowledge of Human Resource functions and responsibilities Federal Laws and Regulations regarding employment experience and/or training.

Regulatory Compliance Coordinator -
Paralegal/legal assistant experience required.  Experience conducting and analyzing legal and factual research including law sources such as statutes, legal code, federal regulations and other legal documents required.  Previous experience organizing and summarizing information for easy processing by staff members at all levels required.  Experience analyzing legal documents for accuracy, including proofreading for contextual, grammatical, or typographical errors required.  Experience interacting on a professional level with attorneys, other organizations and facilities, and internal management required.  Three (3) plus years of education, training or work experience to perform substantive legal work that requires knowledge of legal concepts, principals and practices that is customarily, but not exclusively, performed by an attorney preferred.

Healthcare Relations Representative-
Bachelor's degree or equivalent required, minimum two (2) years of documented sales success, preferably in selling services to medical professionals within the Jacksonville area, clinical background, strong communication skills with ability to listen, computer proficiency with Microsoft Office, customer relationship management and sales force automation systems, current state of Florida driver's license with driving record acceptable to agency insurance carrier.

Housekeeping Supervisor-
High School diploma or GED equivalent.  Prior custodial and housekeeping experience.  One (1) to two (2) years supervisory experience.  Current State of Florida Driver's license with driving record acceptable to Community Hospice of Northeast FL insurance carrier.  Reliable transportation that allows employees to shift work locations when needed.

RN - Home Care
Current licensure as a Registered Nurse in the state of Florida, CPR certification, minimum of two (2) years nursing experience required and recent Home Care experience preferred, current state of Florida driver's license with driving record acceptable to agency insurance carrier, reliable transportation.

RN - Inpatient
Part Time, Nights, 7p – 7a, 72 hours biweekly. Under the direction of the Inpatient Facility Patient Care Administrator or designee, performs a wide range of nursing activities in the various Community Hospice of Northeast Florida inpatient facilities, participating as a member of the interdisciplinary team in providing end-of-life care/support for patients and families alike. Possession of a degree from a diploma nursing program, 2 year Associate Degree, or Bachelor’s Degree in Nursing. Must possess current CPR certification. Current licensure as registered nurse in state of Florida. Minimum of two (2) years nursing experience. Current State of residency driver’s license with an acceptable driving record. Reliable transportation.

RN - Wound Care Nurse
Full Time, Days, 8:30a – 5:00p. Under the general direction of the Director of Inpatient Care, this position is responsible for the coordination and educational support of the interdisciplinary team which includes: nursing and hospice aid services in regard to wound care education, treatment and training. Responsibilities include but are not limited to, utilization review and evaluation of staff and coordination of team activities to ensure quality, cost effective patient care. Make recommendations and assists in formulation of team objectives, policies and plans. Possession of a degree from a nursing program, 2 (two) years program or a Bachelor’s degree in nursing. Three (3) years of clinical nursing experience, with at least two (2) years hospice nursing care. Previous Senior Nurse experience preferred. Certification in Wound Care.

Psychosocial Specialist
Acts as part of the interdisciplinary care team and manages social service needs of a caseload of patients and families. Coordinates all social service interventions and referrals, including needs assessment, supportive counseling, networking, resource procurement. Contributes to the smooth function. MSW from an accredited school of Social Work and three (3) years direct service experience, attends the Interdisciplinary Care Team (ICT) and advocates for patient/family needs with the team and community at large.

Associate Medical Directors
Under the direction of the Chief Medical Officer, the Associate Medical Director shall be a physician licensed to practice medicine in the state of Florida. The Associate Medical Director has responsibility for medical direction of the hospice patient’s care and treatment, and consults and cooperates with the patient’s attending physician. The Associate Medical Director also educates community physicians and others engaged in healthcare services regarding the hospice program and its potential benefits to patients.
Requirements: Doctor of Medicine or Osteopathy degree from an accredited medical school with a current Florida license. Hospice and Palliative Medicine Certification by the American Board of Hospice and Palliative Medicine is desirable within 24 months of hire (if not already certified). Experience with oncology, home health or hospice care delivery is desirable.

Infection Control/Employee Health Coordinator
Graduate from a recognized professional school of nursing; BSN preferred.  BSN preferred.  Five (5) years of hospital/healthcare experience.
Experience with data collection and analysis, and regulatory and accreditation standards related to Infection Control.  CIC (Certified Infection Control) certification preferred and required within the next six (6) months of employment.  Minimum of two (2) years of experience in acute care setting, oncology, or home health care setting; or one (1) year med-surg experience.  Knowledge of OSHA and CDC regulations related to Infection Control and Employee Health.  Current state of Florida driver's license with driving record acceptable to agency insurance carrier and reliable transportation.

Team Physician, FT
Hospice physician needed for Polk/Highlands/Hardee Counties to provide direct medical care as needed to patients in their homes, nursing homes and in the hospital.  You will work with admission staff on certification of terminality and provides advice regarding the initial plan of care for newly admitted patients.  You will also serve as attending physician to patients as requested by them and provides face to face visits as required by Medicare in a timely manner in conjunction with ARNP at Medicare-determined intervals.

Requirements: Licensed as a physician in the state of Florida; licensed/board-certified in primary care, oncology, or other approved medical specialty, having maintained requirements for ongoing certification.

RN Clinician, FT & PT – Admissions Department
The hospital Clinician works closely with the patient and family and the medical staff in the hospital setting. The Clinician facilitates discharge planning to the home setting prior to admission. The Clinician also monitors current hospice hospitalized patients.

Social Worker

(PRN position)

Description:
Provides full range of hospice social work service and bereavement support to patient and families primarily in the inpatient care center setting, as well as in the home and institutional setting when providing assistance to team social worker coverage. Reports to the Patient Care Manager of the inpatient care center and will maintain good communication with both the senior social worker of the field team and Administrator of the field team in order meet the ongoing needs of patients and their families regardless of geographic location. Works with interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment of and support to patients and families. Serves as liaison with other community resources; assists staff, patient’s physician and volunteers in understanding significant social and emotional factors affecting a patient and family during terminal illness. Provides assessment for normal and complicated grief and provides brief bereavement counseling as appropriate.  Works assigned on call shifts.  Due to business needs, may be required to work different shifts, teams and location.
 
Current license as a Clinical Social Worker preferred. If no license, must be eligible for license and must be registered as a Registered Clinical Social Work Intern (RCSWI) with state. If not registered as RCSWI upon hire, must do so within 90 days of hire. Must obtain license as a Clinical Social Worker within 3 years of hire.

Master's in Social Work.

1-2 years experience in field.

Requirements: Doctor of Medicine or Osteopathy degree from an accredited medical school with a current Florida license. Hospice and Palliative Medicine Certification by the American Board of Hospice and Palliative Medicine is desirable within 24 months of hire (if not already certified). Experience with oncology, home health or hospice care delivery is desirable.

The Admissions Nurse is responsible for coordinating admissions to and discharges from the hospice program.  This necessitates coordinating intakes, assessing the needs of the potential clients, delegating or performing the intake, admission, assessment, transfer or discharge.  Inherent in the role is a close working relationship with the Medicare Billing Coordinator to determine coverage for services and ongoing follow up to determine the appropriate utilization of services.  The position requires at least five (5) years actual nursing experience, including basic nursing skills, psychiatric nursing, and medical/surgical hospital experience.  A fundamental knowledge of psychosocial dynamics concerning the patient/family unit is also required.  Must have a current State of Florida RN license and a valid AHA Basic Life Support Certification.

SOCIALWORKER/RFT, BROWARD COUNTY

The Interdisciplinary Team Social Worker/Counselor is responsible for providing social work services (counseling, advocacy and community referral) to patients and their families having difficulty in coping with multiple aspects of terminal illness. The Psychosocial Counselor is responsible for providing social work services (individual and group counseling, advocacy, and community referrals) to hospice patients and families as well as non-hospice individuals in the general community. Services are delivered through our Bereavement Department and our Caregiver Program.  The counselor will also develop and present formal educational programs and lectures for the community; be involved in the ongoing development of bereavement and caregiver services; and participate in the reporting of bereavement and caregiver service outcomes.  The counselor will also participate in our bereavement camp for children.
Masters degree in social work from an accredited college or university required. Minimum of two years professional experience in mental health and with the bereaved.  Valid CPR-BLS certification required. Florida license in clinical social work preferred.

SPIRITUAL CARE COUNSELOR/ RFT
PSYCHOSOCIAL

The Spiritual Care Counselor is an integral part of the Interdisciplinary Team, offering support and counseling to both the staff and the patient/family units. He/she functions primarily in the areas of healing, guiding, sustaining and counseling and is comfortable with the uniqueness of each individual’s beliefs or lack of them. The Spiritual Care counselor is an active participant in the bereavement program and is available for community education. Must be a graduate of the seminary program or certified in clinical spiritual education. A good working background in theology, sociology, psychology, ethics and philosophy is desirable. A basic understanding of and openness to various belief systems is required. Must have a valid CPR-BLS certification.

SOCIALWORKER/PD

The Social Worker is responsible for providing social worker services to patients and their families to assist in resolving social and/or emotional problems which are or are expected to be an impediment to the patient’s medical condition.  The Social Worker is an Interdisciplinary Team member. Provides leadership, performs complete medical record audits, and participates in QI projects and activities.  Assists with staff education. STAFF DEVELOPMENT EDUCATOR/ RFT
EDUCATION

The Staff Development Clinical Nurse Educator is responsible for providing skilled and highly technical education and supportive services to the clinical managers & staff, non-clinical managers & staff, volunteers, students for multiple healthcare disciplines and external agencies as necessary to promote the mission, vision, and goals of Hospice by the Sea.  Typical duties include researching, developing, updating and presenting educational programs in a variety of settings including but not limited to new employee/volunteer training programs and development. Must have previous acute care nursing clinical experience, which demonstrates extensive knowledge and understanding of disease, nursing process, and procedures. . Provides health office guidance through the administration of tuberculin skin testing, TB mask fit testing, tuberculosis education of staff and volunteers. Bachelor’s degree in nursing required with a Master’s in nursing or other relevant field preferred – with at least 5 years of current active performance in a similar role in a healthcare environment. Hospice experience preferred. Evidence of positive interpersonal (teamwork), organizational skills, time management, communication and the ability to work independently with minimal direction in a complex environment with deadline pressure is required. Must possess strong computer skills in Microsoft office – Word, Excel, PowerPoint and Outlook - and have the ability to utilize electronic and digital devices to produce and present educational programs. Current experience with teaching staff the use of an electronic medical record (EMR). Prior experience with the McKesson Horizon EMR a plus.  Must have public speaking or teaching skill/experience.

ACCOUNTING INFORMATION ANALYST / RFT
FINANCE

The Accounting Information Analyst (AIA) is responsible for the accuracy of enterprise accounting information gathered and disseminated for timely decision making. Using requisite knowledge of general business, coupled with skills in business information processing and measurement, the AIA will manage by analyzing large volumes of accounting and related data, and use database management tools. The AIA will test and rely on internal systems and controls, and be accountable for maintaining automated and manual general ledger account schedules, reconciliations, detailed account analysis, and postings of accounts payable (AP), purchasing (PO), payroll (PR), cash receipts (CR), donors, point of sales (POS), fixed asset management system (FAS), and integrated bank reconciliation modules (BR) within the company’s accounting enterprise resource system (ERP); and provide the crucial link to the separate systems that require updates in a rapidly changing environment. A Bachelors degree in accounting is required (Masters Level desired). Experience with account reconciliations, analysis, and reporting using data downloads and electronic businesses (E-business) to the web and other systems. Strong experience in month end entries from enterprise and disparate systems with strong emphasis on automated journal entry preparation, documentation, and upload file format expertise. Must be skilled with other computer applications, including Microsoft Excel, Access, and Great Plains (preferred) or other ERP software, workflow software, and electronic documentation management systems (EDMS)

ASSOCIATE DIRECTOR OF SPECIAL EVENTS, RFT

The Associate Director of Special Events is responsible for supporting the day to day operations of the Special Events Department of the Hospice by the Sea Foundation and direct support to the Director of Special Events. The Associate Director of Special Events will manage a number of diverse special events at any given time; outstanding organizational skills are of the highest importance. These skills must also be matched with a high degree of objectivity and creativity.  In addition, because special events requires coordinated efforts and requires skills from a number of involved individuals, the ability to assist the Director of Special Events to manage personnel and volunteers from diverse backgrounds is highly important.  The position will also manage heavy volumes of volunteers and staff at the Director of Special Events discretion.  Therefore, this position must be able to relate directly to volunteers and staff and be able to identify, recruit and manage those top individuals on an ongoing basis. Because the position represents a first-line of contact between the general public and the Special Events Department, outstanding interpersonal and communication skills are essential.  The successful candidate will have a bachelor’s degree in business administration or related field, with 5+ years of experience in all aspects of special event’s creation, execution and management, as well as fundraising related activities. Extensive computer and typing experience including a solid working knowledge of database software, including MS Outlook, Excel, Word, PowerPoint, and Raisers Edge is required.

The Director of Donor Relations and Stewardship is a senior member of the leadership team of the Foundation and is accountable to the Executive Director. This position has management responsibilities for a robust program of recognition strategies and will prioritize short and long term donor relations and stewardship goals.  The person in this position is expected to develop and implement plans for the Donor Relations and Stewardship program. This position will also provide assistance with donor services such as gift acknowledgment, gift recognition and appreciation, accountability reporting, which in turn supports donor cultivation. CNA/HOSPICE AIDE, FT
Location: South Brevard County
Provide personal care services and emotional support to hospice patients.  Personal care services include, but are not limited to, personal hygiene, assistance with activities of daily living, transfers, and at times, light housekeeping. 
Requirements: Valid Florida Certified Nursing Assistant.  Two to three years experience as Certified Nursing Assistant or Home Health Aide.

HOSPICE PHYSICIAN, FT
Location: Brevard County
Position reports to the Medical Director, in collaboration with the IDT and attending physicians, to manage the medical plan of care for enrolled patients.  The Hospice Physician provides a range of services in the home and facility setting, including face-to-face evaluations, consultations for pain and symptom management, provision of general end-of-life care as the primary attending and education of community members, attending physicians and facility staff members.  Full-time, with rotating call schedule for nights and weekends.
Requirements: M.D. or D.O. degree with license to practice in Florida.  Certification by an ABMS approved board or osteopathic equivalent.  HPM Board Certification preferred.

INTAKE COORDINATOR, PT                                      
Location: Brevard County                                                   
Responsible for the processing of all incoming patient referrals.  Position works closely with the Nursing Department, Admissions Registered Nurse, and the Admissions Manager to ensure timely processing of all referrals.  Position serves as a resource for questions/issues from the health care community and HOSF staff.  Part-time.
Requirements: High School Diploma; College Degree preferred.  Excellent oral and written communication,
clinical and administrative skills.  Able to work collaboratively in a positive fashion with other health care
professionals.  Able to prioritize varied projects.  Dependable, flexible and tactful.

MASSAGE THERAPIST, PT
Location: Brevard County                                                  
Responsible for providing massage therapy to hospice patients according to the written physician’s order based on the nursing plan of care.  Part-time.
Requirements: High school graduate or equivalent.  Valid Florida Massage Therapy License.  Previous experience required.  Ability to read, write and speak English, bilingual preferred.  Ability to manage time and workload with minimal direction.

PATIENT CARE COORDINATOR, FT
Location: South Brevard County
Position reports to the Director of Clinical Services. The Patient Care Coordinator is the leader of the Patient Care Team and responsibilities include:  supervise, evaluate and coordinate the various members of the Interdisciplinary Team; directs patient care services for the team, and serves as patient advocate and coordinator for health care providers in the community who are involved in the care of the team’s patients.
Requirements: Associates Degree in Nursing; Bachelor degree in Nursing preferred.  One year supervisory experience of like or similar position in hospice; or 2 years successful supervisory experience or equivalent in health care organization. Certification in Hospice and Palliative Nursing preferred.

SOCIAL WORKER, FT                                          
Location: Brevard County                                                   
Responsible for identifying the psychosocial needs, goals and interventions of patients/families. In cooperation with the Interdisciplinary Team, a range of services may be provided including, but not limited to: assessment, counseling, advocacy, education, resource and referral services, and discharge planning.
Requirements: Masters Degree in Social Work from CSWE accredited school required. Education may be in a related field, such as counseling, psychology, sociology, gerontology. Position requires ability to work flexible hours. Experience in counseling and/or work with aging. Must be able to work independently and in an interdisciplinary environment.

VOLUNTEER COORDINATOR, FT                                        
Location: South Brevard County                                                   
Responsible for overall coordination of the volunteer program of Hospice of St. Francis, Inc. and the supervision of all hospice volunteers. 
Requirements: High school graduate or equivalent; Bachelors in Human Services preferred. Five (5) or more years experience in positions with responsibility for administrative tasks and/or community service.  Two (2) years experience in a previous supervisory role of volunteers may be substituted for five years of administrative.  Ability to work independently, good organizational skills, tactful with others and coordinate several tasks at same time.

RN On Call (Weekends) Palm Coast, FL
Provide skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with regulations by the State Board of Registered Nursing and company policies when called upon after hours. Responsible for assisting with the development and management of sales opportunities with potential hospice home care customers throughout the U.S. The Bus. Dev. TideWell is an innovative, not for profit organization in our 31st year, now serving over 1250 patients within a four county service area in Southwest Florida.   Developing programs will enhance access for additional patients.

Administrative Director of Grief Education & Support Services

Responsible for the Tidewell Hospice bereavement program and related support services. Accountable for establishing and achieving the strategic objectives of the organization as it directly relates to the department while ensuring the highest quality of grief education and support, before and following death, to the organization’s patients, their families, and the community.

Promotes and practices the mission and values and follows the policies and procedures of Tidewell Hospice.

Researches and develops new and effective ways to meet the needs of the griever.  Conduct feasibility studies and create business plans to integrate needed programs for Tidewell Hospice and the community.

Evaluates, develops and implements bereavement programs in accordance to the strategic goals of the department; monitor program compliance with Federal and State regulations, and Tidewell Hospice policies and procedures.

Oversees the grief specialists; accountable for hiring, supervising, educating, performance coaching, disciplinary actions, and completion of performance reviews. 

Develops partnerships in the community and participates in outreach and education activities.   Offers and coordinates memorial services, bereavement support groups, and related activities for families, caregivers, and the community at large.

Develops objectives, generates monthly reports and manages department budget.

Educational and Professional Requirements:

Bachelor's degree in social work, psychology or equivalent theological degree from college or university required; Masters Degree preferred.

Minimum of 5 years experience with direct counseling of grief and end of life issues.

An equivalent combination of hospice grief counseling and support groups experience and education may be considered as meeting the Education/Professional requirements.

Previous Hospice experience and Certification in Thanatalogy preferred.

Minimum five years of supervisory experience preferred.

 

Administrative Director of Marketing/Communications

Responsible for leading direction for planning, development and implementation of marketing/ communications projects for Tidewell Hospice and Tidewell Health Services, LLC. Manage intra-department and/or inter-department projects from conception through development and implementation to ensure public representation, measurement and success.

Establishes ongoing relationships with media outlets with initiation, planning and execution of comprehensive plan developed in collaboration with executive team. Monitors and reports results to EVP/Chief Philanthropy Officer. Responsible for negotiating media contracts, building and monitoring media budget.
 
Develops, directs relationships and manages speaker’s bureau.  Plans and implements promotional opportunities for internal expertise/representation to speak within the communities Tidewell Hospice serves. Directs Community Relations liaisons and associated staff members to ensure speakers bureau is appropriately utilized and exploited.

Directs program, implements tracking procedures, and monitors and measures Tidewell’s Community Benefit reporting as provided in Tidewell’s service area. Prepares and presents monthly Community Benefit reports for Tidewell Hospice and Tidewell Health Services, LLC.

Represents Tidewell Hospice and Tidwell Health Services at public events and in partnership opportunities as appropriate.

Directs Communications Department in support of business and community development, public relations, communications and marketing initiatives.

Develops, implements, monitors and improves systems and procedures necessary to the smooth implementation and operation of business and community development, public relations, communications and marketing functions.

Keeps Organization informed of the developments in the fields within the scope of the business development and community affairs division, not-for profit management, Community Benefit and governance to report to Executive Vice President to help the Organization operate with initiative and innovation.

Educational/Professional:

RN Admissions: Evening shift variable hours.  As part of the Interdisciplinary Team (IDT), the Admissions RN conducts the initial admission process for patients referred for hospice services. The Admission nurse initiates the plan of care based on identified immediate needs, and communicates with the IDT. Current Florida license as RN, or current licensure from another state plus a temporary Florida permit.  Valid driver’s license and automobile insurance in the State of Florida.  Reliable transportation to meet visit schedules. Minimum two (2) years RN experience where physical assessment skills were utilized.  Must be an RN graduate from a state approved school of nursing.  Bachelor’s degree in nursing preferred.  Current (BLS) CPR certification required.  Prior experience in oncology, home health, Hospice or palliative care setting is preferred. Must be able to handle sensitive issues appropriately and with sound judgment, maintain professional boundaries, and maintain strict confidentiality.  Must be able to effectively plan, organize and prioritize work.  Must be self-directed and be able to work well independently in the field.

RN Clinical Manager-Fort Pierce location
Under the supervision of the Division Director, the Clinical RN Manager is responsible for planning, implementation, delivery and supervision of direct clinical services provided to patients and families, coordinating the interdisciplinary team services, and the effective and efficient use of resources.  A minimum of three years of nursing experience in home health/hospice setting within the past five years and one year of managerial experience required.  Current Florida RN license required.

VNA HOSPICE OF INDIAN RIVER
The VNA of the Treasure Coast provides home healthcare services — including Medicare-reimbursed and private-duty nursing; home health aides; physical, speech, and occupational therapy; in-home companions; and personal transportation — throughout the Treasure Coast.
