Nursing homes and hospices have been working together since the 1990s, but it doesn’t mean their relationship and coordination of care has matured and melded. Although there are mirrored contracting requirements for each, that doesn’t ensure a true blending of the care plan and services delivered. This is partly due to the fact that hospices are guests in nursing homes. As guests and partners, hospices need to know – and observe – the house rules. This session will list and review the nursing home conditions of participation most relevant to hospices, with an emphasis on the quality of life (F-309) and advance care planning (F-155) tags.
