This web conference is the third in a series related to payer-provider partnerships. Highmark, which received the Gold Fine Award in 2013 for its Advanced Illness Services, has implemented several successful strategies for improving access to palliative care and supporting members, their families and caregivers in dealing with serious or chronic medical conditions. The web conference will focus on two of these strategies: the Quality Blue Hospital Pay for Value Programs and the Advanced Illness Services Program. Participants will learn how these programs developed, the key milestones achieved and some of the challenges encountered along the way.
