The demand for hospice programs continues to grow due to CMS hospital readmission penalties.  It’s a challenge for agencies to meet the demands of onboarding new staff or providing reorientation for other staff.  This webinar will focus on the Medicare and Medicaid Hospice Conditions of Participation, hospice philosophy and values, the purpose of the interdisciplinary team, and data reporting requirements.  This webinar will help you provide consistent training for your onboarding program.  This session will address difficult conversations for new staff and identify rehospitalization risks, with the aim of improving agency, patient, staff, and contracted-provider outcomes.
