State Association Partnership Agreement
December 13, 2016

___________________________   ____________________ (Association) desires to participate in the “Confidently Auditing Therapy Documentation: A Primer for Quality Assurance, Performance Improvement & Clinician Proficiency” webinar as a Partner Association.

 

The Ohio Council for Home Care & Hospice (OCHCH) will:

 

  • Provide electronic promotional materials to Association for distribution to its members.

 

  • Invoice the Association a fee of $85 for each registrant from their state, following the webinar. This fee is payable by the Association to OCHCH for each of their paid registrants. Association’s registrants may pay to participate in the program through one of three methods: 1) Payment of registration fee and participation in the live webinar; 2) Payment of registration fee and participation by listening only to the program audio recording (not accessing the live webinar); 3) Payment of registration fee and participation in the live webinar and payment of an additional fee to receive the audio recording.

 

  • Provide all logistical services relative to the webinar, to include handout materials, dialing instructions & web link, CE certification & processing, etc. – directions for CE processing will be provided to attendees with the handout materials.

 

  • Provide Association with an audio recording of the webinar for the Association to distribute (if desired) to their paid registrants per the above definition. The Association may opt to promote only the live webinar, and not the recording.  Association may not sell the recordings after the webinar has concluded, and may only distribute the recording to paid registrants.  Additionally, the Association’s registrants are prohibited from reselling or redistributing the recording.

 

Association will:

 

  • Promote the webinar to its members (and non-members at their discretion) using OCHCH provided promotional materials. Association may set the webinar registration rates and audio recording fee in their state as they see fit.

 

  • Handle all registrations for their participants, and e-mail to attendees the OCHCH provided dialing & web link information and handout materials. Association is responsible for distributing the audio recording to their paid attendees (if desired).

 

  • Three days before the webinar, e-mail to OCHCH (Melissa@nullochch.org) a roster of their participants (including agency name, contact, and phone number). This roster may be updated via e-mail up until 10:00 am Eastern on the day of the webinar, and shall list all paid registrants (regardless by which method they participate).

 

  • Pay the OCHCH invoice within 30 days of receipt.
