Hospice case management is required by mandate of an interdisciplinary group.  However, the key to success in hospice is to actually have an effective team that is managing the patient’s case.  Often hospices have the RN as the core team member, and other disciplines are more peripheral.  Whether you are in the early stages of case management, have been doing it for years, or haven’t even heard of it, this webinar will provide best practices and share others’ experiences.

You’ll learn simple strategies to improve your agency’s case management model.  These will include selection of case management teams, communication between teams, and what and how to discuss and document in IDG meetings, with a focus on goals, improving patient outcomes, admission and recertification conferences, and more!

In this era of extremely stringent surveys by CMS, state, or other accrediting bodies, having an effective case management model will lessen your vulnerability for condition level deficiencies and increase survey results as well as increase patient outcomes and increase customer and employee satisfaction.
