This is driving question behind Florida Hospice & Palliative Care Association’s (FHPCA) 28th Annual Forum Conference “Journey: Mapping Our Past, Paving New Roads”. On May 23 & 24, 2013 expert speakers and seasoned hospice professionals will gather together at the Hilton Orlando in Orlando, Florida to explore what the hospice industry has been through these past 40 years, as well as taking a look at what the future holds for hospice and end-of-life care. Join your peers from around the southeast United States for this end-of-life clinical conference designed for the entire IDT team, and explore where you have been and where you are going on your personal hospice journey.

Continuing Education Information:
In order to receive continuing education credit, the applicant must be a registered attendee of the Forum, complete an evaluation form for each workshop attended and have their Florida professional license information on file with FHPCA. Completed evaluation forms will be collected on-site and exchanged for certificates indicating continuing education contact hours earned at the close of each workshop. Your professional license information, including contact information and license number, must be on file with FHPCA prior to the Forum to ensure accurate awarding of continuing education credit.
Out-of -state attendees and attendees seeking credit for all other Boards/Credentialing Organizations are encouraged to contact their respective boards/credentialing organizations prior to the event to ensure credits are applicable.
FHPCA’s Annual Forum is a recognized continuing education event for the following CEU and CME credits:

When you arrive at the Hilton, you can check in at the Forum registration desk, located on the lower level outside Orange A. There, you will receive your conference bag, name badge and materials. In our ongoing commitment to keep Forum a green conference, printed workshop handouts will NOT be available at the conference event.
